If you know of someone who deserves to have a lasting legacy, or someone who personally changed your life, contact us TODAY!  Selection is at the discretion of the of the Name of Honor organization and the information you provide will be made available online (or to the pre-screened family that chooses to honor your legacy).  Please include the follwing information:

• First Name (Middle Name optional, NO Last Name)
• Nickname
• Hometown
• Picture
• Age
• Year he/she passed away
• Short profile on why he or she deserve to be a "Designated Hero" (750 word max) 
• You may be requested to supplement with recommendations
